AUTHORIZATION TO OBTAIN HEALTH INFORMATION
The Onduo LLC Platform ("Onduo Platform") is a resource made available by Onduo LLC and its affiliates to enable individuals to view and manage their medical information online, and communicate with certain health care providers.
I authorize Onduo, LLC, Onduo Professionals, P.C., other Onduo-affiliated professional entities, and any physician, physician practice, hospital, hospital system, or other health care provider who utilizes the Onduo Platform (collectively, “Platform Users”) to obtain the following personal health information about me from my connected medical devices (including but not limited to glucometers, blood pressure cuffs, insulin pumps and pens), physicians, other health care providers, laboratories, and pharmacies including, but not limited to data from pharmacies in the Surescripts network, and from services that send the data from my connected devices to Onduo including Tidepool and Validic, and other third parties (“Data Sources”). Such data may include my: medical and mental health records, claims data, device readings, laboratory values and results (including HIV-related information), and pharmacy data including medication history, whether created before or after the date of this authorization.
I understand that my personal health information may be collected, stored, and used by Platform Users for the following purposes: delivery of my health care, care management, quality improvement, payment for my health care, research relating to the impact of the services provided by Platform Users on the management of chronic illnesses such as diabetes, hypertension, congestive heart failure and depression, and to share high level clinical outcomes on our website and in marketing mat, and developing the current and future capabilities of disease management systems and related algorithms used in the Platform or in other programs by Onduo, LLC and its affiliates. This authorization will expire three years after conclusion of my treatment by all Platform Users.
I understand that this authorization is voluntary and that no Data Source or Platform User may condition treatment or other services on my willingness to sign this authorization. I may revoke this authorization at any time by notifying Onduo LLC in writing at Onduo LLC, 55 Chapel Street, Suite 10, Newton, MA 02458, Attn: Privacy, or by email: compliance@onduo.com. If I revoke this authorization, I understand that it will not have any effect on actions taken by Data Sources in reliance on my authorization before Onduo LLC received the revocation.

La Plataforma de Onduo LLC (“Plataforma de Onduo”) es un recurso que ofrecen Onduo LLC y sus afiliados para que las personas puedan ver y administrar su información médica en línea y comunicarse con determinados proveedores de atención médica.
Autorizo a Onduo LLC, Onduo Professionals, P.C., otras entidades profesionales afiliadas a Onduo y a cualquier médico, consultorio médico, hospital, sistema hospitalario o proveedor de atención médica que utilice la Plataforma de Onduo (en conjunto, “Usuarios de la Plataforma”) a obtener la siguiente información de salud personal sobre mí de mis dispositivos médicos conectados (incluidos, sin limitaciones, glucosímetros, brazaletes de tensiómetros y bombas y plumas de insulina), médicos, otros proveedores de atención médica, laboratorios y farmacias, incluidos, sin limitaciones, datos de farmacias de la red de Surescripts, y de servicios que envíen los datos de mis dispositivos conectados a Onduo, incluidos Tidepool y Validic, y otros terceros (“Fuentes de datos”). Esta autorización vencerá tres años después de la finalización de mi tratamiento por parte de todos los Usuarios de la Plataforma.
